This policy applies to information collected through the Website. It does not replace our HIPAA Notice of Privacy Practices, which explains how we use and disclose protected health information in providing health care. If this policy and our HIPAA Notice of Privacy Practices apply to the same information, the HIPAA Notice of Privacy Practices controls. Please do not use a general Website form to send urgent requests or detailed medical information. Call 911 for a medical emergency. For questions about your care, contact our office at 631-543-1440 or use an approved patient portal, if available.
